The Traditional Clinical Track (Residency / NRMP Match)
The standard route to full independent medical licensure and practice in the US.
Core Requirements:
- Exams: USMLE Step 1 (Pass/Fail) and USMLE Step 2 CK (scored).
- ECFMG Certification: Primary source verification of medical credentials + OET (Occupational English Test) + meeting clinical skills Pathway criteria.
- US Clinical Experience (USCE): 3–6 months of hands-on electives, sub-internships, or externships to obtain 3–4 strong US Letters of Recommendation (LoRs).
- The Match (ERAS/NRMP): Apply in September, interview from October to January, and match into a residency program in March.
Objective: Full US Licensure | Advantage: Unlimited long-term practice | Barrier: High competition & USCE requirements
The Academic & Research Fellowship Track
An ideal bridge pathway to build a competitive edge for university-based residency programs or competitive specialties.
Core Requirements:
- Postdoctoral Research Fellowship: Secure a 1–2 year research position at a major US academic medical center (e.g., Mayo Clinic, Harvard, Johns Hopkins).
- Academic Output: Generate peer-reviewed publications, present at national US conferences, and build direct networks with Program Directors.
- Transition to Residency: Complete USMLE Step 1 and Step 2 CK while doing research, then apply to the Match with internal institutional backing.
Objective: Academic Portfolio | Advantage: Direct networking & high match odds | Barrier: Usually requires 1–2 years prior commitment
Alternative State-Specific & Industry Options
Pathways outside the standard 3- to 7-year residency training model.
Core Options:
- State-Level Roles: States like Missouri, Florida, Tennessee, and Washington allow doctors with passed USMLE steps to practice clinically under supervision without a standard residency.
- Industry (Pharma/Biotech): Roles such as Clinical Research Associate (CRA), Medical Science Liaison (MSL), or Medical Advisor in US pharmaceutical or CROs.
- Clinical Fellowships: Certain non-accredited or subspecialty clinical fellowships accept foreign-trained doctors directly if they hold home-country board certifications.
